This video details the approach to a perineal urethrostomy in a cat. Initially purse string sutures are placed, the urethra is catheterised, and an elliptical incision is made around the prepuce and scrotum. If the cat is entire, a castration is performed. Soft tissues are dissected towards the ischial arch and a ring retractor can be placed to assist in the transection of the muscles attaching the penile body to the pelvis. The dorsal urethral wall is exposed and incised to the level of the bulbourethral glands, before the urethral mucosa is carefully apposed to the skin. Finally, the distal penis is amputated and the remaining ventral skin incision is closed. It is important to ensure the urethra is patent at the end of the procedure and the stoma is a sufficient size.
